The finale of the 1st stage was disrupted by a crash, of which Matteo Moschetti was the biggest victim. The following morning he did not start, and later that day it was also the end of the Giro for Jay Vine, Marc Soler, Santiago Buitrago, Adam Yates, Andrea Vendrame and Ådne Holter. A large part of the peloton was involved in a major crash, and they were so badly affected that they were unable to continue the race.

Wilco Kelderman became the final casualty of that major crash. He made it to the first rest day but was too battered to start stage 4. Kaden Groves endured a similar fate: the Australian crashed on the opening day in the same incident as Moschetti, continued racing, but withdrew early in stage 4.

Last year, Mikel Landa crashed heavily in the finale of the 1st stage and had to abandon the race with a fractured vertebra. Geoffrey Bouchard also had to withdraw with multiple bone fractures.

That made them the first riders to abandon. In the end, 25 riders would not reach the finish in Rome. Among the withdrawals were Primoz Roglic, Juan Ayuso, Giulio Ciccone and Jai Hindley.
